Getting error while saving the Script Include for first time after upgraded to UTAH

Hi Everyone,

I have been using UTAH since it's available for PDI, I am getting an error while saving the Script Include form after putting in some Script, when I am trying to save for the first time it's clearing the form and throwing an error. Please see the below attachment for the error. Does anyone have the same issue?

And ecently we have upgraded our DEV to UTAH, and i am getting the same error

You show the form when you click the "new" button on the script_include table. Fill in some values.  However I was able to save a record like that, it does nothing though.

I am not sure why this happened though, but this is happing whenever I want to create a script include. My workaround for that is to save an empty form and then give some data into it and then save. Then it works.

I have no idea why you can't save that. it works in my PDI.

I see 5 UI Action records defined for the sys_script_include table in my Utah PDI.  What steps are you using? And what happens if you actually fill in some values on the form?  I suggest that you disable those where "Form action" = true one by one and test.  And see if you can determine which is causing your problem.
