How can I create Work order task Survey which can generate multiple incidents/emails?

Hi,
Within FSM, engineers complete a questionnaire at the end of a job. The questionnaire checks certain things, but then allows the engineer to report a fault and the fault type is dropdown listing 16 or so different issues. The fault type will either generate an incident or send an email to a team to investigate as soon as the questionnaire has been submitted.
We have a requirement to allow the engineer to submit multiple faults within the questionnaire. What is the best way of achieving this? If there are multiple ways, it would be good to hear all the options available.
We have explore having a question at the end, 'Would you like to report another fault?' and then reload the questionnaire. Not sure if this is possible, as then a new survey is generated.

Yes, that's correct. A questionnaire or survey is meant to have a predefined set of questions. If you would like to add more, you need to publish a new version of the assessment to ensure that in flight assessments/surveys are not affected.
In your scenario, it is not straightforward to capture both issue types and their corresponding attachments using a survey or questionnaire.
1)You could create a single attachment field and rely on engineers to provide attachments for each issue they select.
2)You can then use a script to validate whether the number of attachments matches the number of issues selected.
3)If they do not match, display an alert prompting users to provide the correct number of attachments before submitting the assessment.
4)Ask engineers to use a naming convention like Issue_1_short_description for the attachments, so you can easily read and process them when creating incidents.
