How can I move a form from one instance to another? Update set has been corrupted

My update set has been corrupted.   How can I move a Service Portal form from one instance to another?

I attempted to manually select all variables from the Cat Item Variable list view and the same goes for the other items, i.e. Cat UI Policies, and use "Action on selected rows / Add to Update Set"   It appears there were added. And when I import xml to other environment the rows appear to show up.   But the form does not work.

I do have an "add to update set" utility, I will check to see if it is the same one.   The one that we do have in place I used, and no go.
