How do "UR Certified" copy details from created Incident to created UR

I have quite a few catalog items and record producers creating INCs and SCTASKs.  We want to start using the UR certified field, but when checking that box, the UR that is created is not bringing along some of the details we want (Caller, On Behalf Of, Configuration Item as examples).

 

I've been looking around and I can't seem to find where/what I need to edit to get this to work.  The main problem we are having is that when someone submits the form, it doesn't show on their tickets list (we have the ticket list in Service Portal showing URs) since the caller/on behalf of is not being added to the created UR.
