how to add radio button in service catalog variable ?

Hi All,

I want to add radio button in my service catalog variable , do you have any idea how to do that ?

Field label	Field type	mandatory
Do you already have a company phone?	Choice (Boolean)	*	Radio buttons for 'Yes' or 'No', may trigger additional fields or processes.
