How to configure two different Change tasks form

I have two different types of change task. One for verifier and implementers. I need to put the additional implementers field on the implementation plan and the additional verifiers on the review task.

How do i add each field to their respective tasks with out them showing up on the change task form together.

You could use the type field (screenshot below) on the change task. Setting it to implementation for one task and testing for the verification task. Then use a UI Policy to make the fields visible basked on the type field.

Hi Brian, 

so i went to the change task for the implementation and used the uI policy to specify if the task as for implementation but in the UI actions i do not see the new custom field i created called "additional implementor"
