How to create a form with a visual state transition similar to the Problem or Change Request table?

Hi everyone,

I'm trying to create a custom form that includes visual state transitions, like the ones in the Problem or Change Request tables. I've attached a screenshot for reference.

Could anyone guide me on how to set this up or point me to any relevant documentation or examples? Your help would be greatly appreciated!
