How to create a normal change template and request item by using catalog item

Hi, I need to create a catalog item 

Based on the selection of a variable, I need to create a change template or RITM's.

Based on the option I selected on the form the Change template or RITM should be created in the respective table after submitting the request.

Requesting approvals of new change templates and creating requested items (based on change template??) are complete different topics and shouldn't be mixed. Better to keep them separated (and thus close to OOTB) to reduce the efforts and risks.

Generally, we avoid creating templates from SR, from SR you can request a template. Also, to create a change template, much information is required, so you need to get all information from the user, which can make your catalogue item lengthy and complex.

You can use flow, and based on the variable input, you can create task for team to create a template, but again fulfiller team minimum like Description / Short Description/ Risk / Assignment group / Planning information and more to create a template.
