How to Populate an exist file into Attachment in Service Catalog on Portal

Hello everyone, my requirement is populate an exist file into Attachment in Service Catalog on Portal ! Please someone help me ! Thank you so much

Is it something like whenever anyone opens the form on portal. They should see a attachment on the form.

Sorry for my requirement not clearly ! The solution doesn't solve my problem !

In my form have a variable reference to table Incident => All files attachment auto populate depend on variable reference to table Incident
