I am getting 2 request approval ui actions in Normal change form

On change form of type Normal i am getting 2 ui actions Request Approval . for emergency and standard change i can see one Request Approval action.

Can anyone help me on this?

Please check in UI action, is any one created a new UI action with same or copy ? also try to repairr change plugin once.
