If members of a group rejected the approvals, still task creates

Manager approved, if approved then group approval, if members of group approved, then create task, if members of the group rejected, still task creates.

Any idea or tips are greatly appreciated.

Sorry, possibly I can't explain nicely. The issue is when members of the group reject, still task creates that I don't want.
