In which instance do we need to create a change request before changes moved to Prod instance?

Change requests are always create din PROD instance, It is differentiated by the CI you select, Applications have different CI/Service for PROD/UAT/DEV etc. 

If the CI selected is UAT then its a UAT change.

Can we create change requests form service catalog, if yes, please let me know the process how to do that?

Depends on the use case, You van explore Standard Change Requests , which is in simple terms routine changes with Minimal to no risk (Low on the record).

Standard Change Requests can be presented on the Service Catalog and have a similar look and feel to it.
