Is it possible to create a Multiple choice field type on the form as described in the attached screenshot ?

Hello experts,
I have gone through other questions in the community on how to create a multiple choice list on the form, but I could not get a clear answer from those posts. A work around would be to create a List type field, but I would like to know if it is possible to create a Multiple choice field as shown in the below picture(I did some Photoshop)
Please let me know.

Hi,
I'm not really sure what you're asking for...but you can create a multiple choice field, with three choices...but they are bubble dots and you can ONLY pick one choice...OR you can create three checkbox fields, which you can make at least ONE mandatory, but they could pick more than 1 option.
What are you wanting to do here?

Thank you for the quick reply. I need a field on the form, where the end user can select more than one choice. Like in the screen shot, user should be able to select either 1 or more than 1 option.
Please let me know if you have any other questions, ill try my best to explain further.

Hi,
Ok, so for this you'd want to first create a label variable and put your question there...like Access Type or something like that. Then create however many checkbox fields you want, in order, right after the label variable. They will all fall in line with that label. If you don't use a label, you'll see the word: "Options" and a lot of people don't like that so they create a label field, which replaces the word "Options" to whatever they want.
This will allow a user to select one or more than one option that falls under Access Type (for my example).
The next step after this would to decide if this is a mandatory section...or not.
If it's mandatory, let me know and I'll show you how to make at least ONE choice mandatory, but they can still pick more than one.

I will try your approach. I will mark your answer correct once I am able to achieve this.
To make at lease one choice mandatory, I can use UI Policies(Correct me if there is another good way.)
