Modified Copy of "Software End of Life Request Flow" Not Triggering — Original Flow Still Runs

I copied the out-of-the-box flow "Software End of Life Request Flow," (available in SAM PRO) made a few modifications to tasks, and then published and activated it. However, when I create a record, the original flow still triggers instead of my modified version. What could be the reasons for this behavior, and what troubleshooting steps should I take to resolve it?

@VisweshwarR , Check if the OOTB flow is still active. If it is, it will continue to trigger instead of your modified copy. Deactivate the OOTB flow.

The flow is deactivated and confirmed it is active false
