Multiple change verifiers and change implementers

I want to be able to give the ability to enter multiple change verifiers and change implementers so that the change request form reflects the involved individuals.

My first solution  is to add  2 New fields on the form for additional implementer and additional verifier

How can i get these to new field to show on the implementation task and verification task that have been created

Add them as dot-walked fields. Go to your form layout of the task and add the change_request.custom_field to the form.

Im trying to dot walk the change request form to get to the "additional verifier" field but I'm only seeing change request>parent option. is this the correct selection. if so how do i open it to see the fields associated with the form to bring it to the selected area

If you are linking through the 'parent' field, than that is indeed the correct field.
OOB it would just be the 'Change request' field from where you can get the related fields:
