My Approvals Widget Shows "Loading approvals..."

Hey everyone!

 

About  year ago, I created one catalog item with an approval flow that displays the approval request within the approver's My Approvals widget.  The approver is able to view the Options and click on either the Approve or Reject button.  Once the approver clicks either button, the flow processes as normal.

 

I recently created second catalog item with a second approval flow and it only shows the "Loading approvals..." message in the approver's My Approvals widget.  No Options, no buttons appear for the approver.

 

Notation:

 

Old Approval Flow Name: CDPR Email Retention Exemption

New Approval Flow Name: CDPR - Application Request

 

Troubleshooting steps so far:

 

1. Compared both the old and the new approval flows and they appear to have the same flow properties (please see attached images).

2. Impersonated a standard user and began an approval for both the old and the new flow.  Only the old flow approval appears in the My Approvals widget (please see attached image).

Question: Normally, I would check to see if the My Approvals widget is broken, however if the old approval displays the approval in My Approvals widget, but the new one does not, how can the My Approvals widget be broken? 

 

Question: I would also check to see if there was an issue with permissions, however both the test user and their supervisor are exactly the same, so why would there be a permission difference if one flow works and the other does not?

 

3. Reviewed the Execution in Flow Designer for both submitted tickets (please see attached images).
