Need help on Multi-Row Variable Set uses in Flow Designer

Hello,
1. I created a Catalog Item with 1 Multi-Row Variable Set which contains a Sys_User reference and a single line text field.
2. Created a Flow with the following Actions:
3. Submitted a Service Request with 1 user in the Multi-Row variable set
4. The problem I'm having is it creates 2 records. It should only create 1 record that are in the Multi-Row Set
Could someone please help?

What do you mean with "it creates 2 records"? Which records in which tables?

I totally forgot to attached the 2 records system created. RITM0010152 - Abel Tuter is the Multi-Row record and RITM0010151 - System Administrator is the system created.
It is almost 12:00 am in the United States and I'm still trying to figuring out the what caused it to creates 2 records. I hope you can provide suggestions how I can fix it.

Could you please provide suggestion on how to do this in Flow Designer?
We have 1 Multi-Row variable set on the Catalog Item. Based on the number of records on the Multi-row set,
we need the system to creates "1 Request, 1 RITM and 1 Task" for each row/record.
