notification toggle not working properly

notification toggle is not working inconsistently for custom device even after making the notification subscriable
when user toggle and refresh the notifications are not saving? Please suggest

I make the notification subscriable for em_alert table when I toggling enabling them for SMS custom device and refresh the page then the enable thing is not saving: Please see attached screenshot for reference:

After refreshing page:
Note: There is no duplicate records in cmn_notif_message table:
sometimes its working sometimes not that's the issue. Please help as it is escalated one.

Hi,
I already checked this KB. user has snc_internal role and snc_external role is excluded. Please suggest something else.
