Order Guide Rule

I have a simple order guide rule that if a Yes/No question is No, it should result in a catalog item coming up on the "Choose Options" screen.  I have deleted and recreated the rule but it will not launch the item.  The item doesn't have any restrictions on it and it is active.  Each of the rules have a unique order number.  

Anything I can try to troubleshoot this issue?  Should I remove all the rules except the one that's not working?

The order guide does include a sequence but even if I remove the sequence entirely, the rule still does not work.

Rule is looking good. 

 

1. confirm same variable (yes/no) you are selecting from order guide form in order to launch catalog item

2. confirm other order guide is working fine , only your one is not working OR all order guide is not working.

One of my colleagues had chosen to "hide on service portal" so that's why it wasn't coming up in the order guide!
