I'm looking for to configure something like below image, we have customer survey after any incident is closed or request is close complete, stakeholder wants to see responses like below image.

You need to bulild a report on asmt_metric_result table and create SAT and DSAT % report type. Did you start with it or your struck at some point ? 

if your yet to start then this info will be your starting points to build it. you will need to create indicator for positive and negative feedback and show % accordingly.
