Personalized List Column setting is gone

Hello there,

A user reported 'Personalized List Columns' setting for incident has gone (the user did not reset the setting to defaults). It seems like this issue happens time to time. Is there any way I can restore the setting? Also, can someone tell me where this setting is saved?

Any advice would be appreciated. Thank you!

I found 87 records (incident) with User = empty in our instance. And it's been created/ updated by different users. Do you know which operation trigered to create those record? If user=empty record is a root cause of this issue, I'd like to prevent someone creating the record.
