Populate text box after resolving incident in sp portal

Hi,

I have a requirement like, When I click on 'resolve' in incident in SP portal, then after it should populate a comment box with the title 'Please let us know the reason your issue no longer needs investigating'
AND this comment is populated as both an Additional Comment and the Resolution Notes on the incident record.
