When I create a new incident survey and I customize it by adding some code in the OOB UI page called assessment_take2. When I tested, it triggered an email notification, I went to system logs -> email, then opened up a preview email and clicked the take a survey button and then I received this error as the screenshot shown above. I googled where that error came from and I only see 1 KB that said it's from assessment_take2. So I followed their instruction by reverting it to the original but the error still remains. I already added myself as admin_survey.
Make sure your assessment is Publish
