We use can log only Standard change from Portal and that is recommended.  For that you need to create the standard change template and map that to record producer. 

If you want to create Normal change that is not recommended.

But you you write the BR, standard change is OOTB way.

Is that record producer for change request or is it creating any other task record and then you want to creat change task.
If thats the case write a script in record producer to create a change request and give your desired inputs to the record using scripting.
