Hi @tGhadage  

For this you need to create the Database view,  as task are on change task table and change on change request table.

You need to create DB view like

Instead of task sla , use change_request table.

have added the database view on the report regarding the condition what shall I use to get the data shown in above table format ..

I mean get the change task assigned for the particular change request.

You will not get in same format as you need,
