RITM stuck in waiting for approval and not creating task

I have created a Catalog Item for a leavers records, I just need to create a RITM with a list of tasks.

When I submit the Item, the REQ has an approval state of 'not yet requested', request state of 'pending approval'

RITM is listed on the REQ when navigated it shows 'Waiting for approval' but no task created.

When I run the test in flow designer it gives the message following message : -

As per your flow there no any approval before task generation so, task needs to be generated directly without any approval state changes (if no logic present for the same in other script or workflow). 
As per the error present in the image, please check in step no. "3" and "4" that is there any mandatory field is blank on RITM update activity. If yes, please fill them, save it and run again by generating new request.
