are you setting the state during creation?
Is this happening for all catalog tasks or only for catalog tasks for specific catalog item?
Did you check the default value for state field on sc_task?

hi Ankur,
YEs i am creating a new workflow and attaching to an existing catalog item. For the specific catalog item i am facing this ( upon giving the selections all the task are going to unsuccessful state). i checked default value is 'assigned' only.
