Service Catalog 'Order Now' and 'Add to Cart' not working

When I click "Order Now", the status updates to "Submitting..." and hangs there for a few miutes. Once the page updates it shows the requested item form with no record and then redirects me to the service catalog item form that I just submitted.
When I click "Add to Cart", the portal displays "The item is successfully added to my cart", but it does not actually show up in the cart.
These are my console warnings:
Any ideas on what's causing these issues?

Are you using the OOB widget (or) customized widget?

I tried using both my customized widget and the OOB widget. Still the same issue unfortunately. Any other suggestions?

Hi,
This looks like a customized widget. I would suggest revert it to OOB widget and then try again to see if the issue persists or not.
Widget Responsible for Controlling Order Now and Add to Cart button:
Navigate to widgets and then search as Name = SC Catalog item
Open the Record and scroll down to Related List named "Version" . If Version is not present then add it as a Related List using Configure--> Related List and then revert it to OOB Version.
Also, was there a use case on why this code widget has been customized?
