Service Catalog 'Order Now' and 'Add to Cart' not working

When I click "Order Now", the status updates to "Submitting..." and hangs there for a few miutes. Once the page updates it shows the requested item form with no record and then redirects me to the service catalog item form that I just submitted.

When I click "Add to Cart", the portal displays "The item is successfully added to my cart", but it does not actually show up in the cart.

Any ideas on what's causing these issues?

Can you post a screenshot of your catalog form after reverting it to OOB for the widget "SC Catalog item"

After reverting the refresh your catalog form and then share it here.

This is the catalog form. 

And when I click order now. It will flash the request page for a second, then redirect back to the catalog form.

*One other thing to note is that I am not having this issue in my Dev instance. This is only happening in my QA instance.
