The number of  reference qualifier depends on the number of reference fields you have in the form. 

Ex:

If you have 1 reference field and 1 List collector field, hence you can configure 2 reference qualifier for those fields.

There can be as many conditions as you want in a reference qualifier. You can have a reference qualifier to a reference field or a list collector field.
