In my company instance so many stale CI Remediation tasks created for classes without a staleness rule

1. Please guide from where this Stale CI remediation task is created and how to stop this classes without a staleness rule..?

2. What is staleness rule and how to create this ..?

Quick question please, IM not able to access CMDB Health Metric Preference from Navigation menu. 

Is it accessible only with the URL /cmdb_health_metric_pref_list.do ??
