State: undefined and Resolved

We have an intermittent problem being reported by some of our fulfillers.  When they change an incident to Resolved they are sometimes getting the state as "undefined Resolved", though so far they only noticed it during that state, when closed it only shows closed, and they don't even always see it this way.  Here is how it appears in the Service Operation Workspace:

Anyone have any good places to look for an intermittent issue such as this?  Any suggestions are welcome!  Thank you in advance.
