System is creating new records automatically with empty "Created" and "Created by" fields

I'm dealing with a very strange issue I've never seen before. We have a custom table where you can save a form as a "Draft" (current status = draft). Several fields are required to be filled out before you can even save it as a draft. However, we've noticed that there are a bunch of empty Draft records being automatically created in the system. The record is active, but not a single field on the form is filled out. The strangest thing is that the "Created" and "Created by" fields are empty, and there is no history whatsoever on the record. This makes it extremely difficult to pinpoint when and how these tickets are created. 

Does anyone have any idea of what could cause a record to be automatically created without a "Created" or "Created by" date? This is happening in our DEV, TEST, and PROD instances. There doesn't seem to be any pattern for when these are created.

Is that a PDI or a paid instance? If it's paid, I strongly recommend to consult HI for help. BR Dirk

I actually did figure out the issue! It wasn't a display BR, it was a Run Script in the workflow. We have a parent workflow and a child workflow, but depending on some field value in the form, sometimes a child record isn't created. Towards the end of the workflow, there was a script to "update" the parent, which was confusing the system if the record didn't have a parent, so it was simply creating a new one. The "updated by", "updated" and "created" fields were blank because whoever wrote this also made it so that those fields didn't get updated.

That is great news, that you were able to figure out.

I personally would like to get a chance to review this scenario in detail, because I guessed, that this is "impossible".

So, is it possible, that you can post some screenshots etc. here, so that I can see the steps, that lead to that situation, and maybe screenshots of the records without those values.
