Template Field Read Only Issue.

Hello Community,
I had one requirement where i need to make the Template filed read-only for everyone in the Change Request form. It should allow only Change Managers and admins to edit them regardless of the state of the change.
We implemented a on-load Client Script. It is working properly, only issue here is whenever a change- coordinator applies the template from the toggle template bar, the name of the template is not visible on the template field. Due to this, some CTASKS are not getting triggered.
Very much appreciated your guidance on this.

here is the screenshot, whenever we select template from the toggle template bar for the change request, it is appearing as empty. even though we have selected it.

After seeing the script this screen shot looks like it is a custom field, what this field is doing? OOTB also has some template fields which can be used.
