I have enable the Polaris dark theme as suggested but still don't have the Theme option available under Preferences.
Any idea as to what would cause it not to display?

Hello,
Yes seems to be a instance issue I am also in the same patch as you but the issues does not occur for me it would be better to raise a HI ticket with ServiceNow.

Creating a new theme and setting it as the default is the cause, that is if it does not include a Dark variant. Once I've added the variant and refreshed my browser the Theme option showed up.

Let's disable both the Polaris Experience and the Polaris Dark Theme, and enable one by one.
Set the Polaris Experience (glide.ui.polaris.experience) property to true first.
Refresh your home screen and confirm the view has changed.
Next set the Polaris Dark Theme (glide.ui.polaris.dark_themes_enabled) property to true.
Refresh home screen.

Hello,
Please make the property false and then true again.
Then logout and login sometimes it can be session issue
If the above does not work use cache.do in the navigation.

Thanks, Deleted all my preferences, plus cleared cache then logged out and back in but still no luck. Maybe cause for a support ticket.
