UI Action is not picking the condition

Hello Team,
Ui Action is not picking up the condition where it calling script include , please help me to get this issue resolved
ideally it should skipped the Risk assesment for RFC change , poreviously it is working but not now

it looks like an OOTB UI action but custom condition

you will have to debug that function 1 by 1 by adding gs.info() and see where it's failing

it is not OOB, it is Custom
