UI Actions not available when creating new record on Portal

Pretty new to Service portal, so working through some basic issues.

 

I have custom tables with Global and Table ui actions.  From the Portal, I have a simple list widget, allows the user to open a form to view the record, or if they click VIEW ALL they can see all records or click NEW to create a new one, however, there are no UI actions on form, not even OOB save/delete.

 

However, it so happens that I have the same for other custom tables, and the UI Actions are available, both custom and OOB.

 

What would cause this?
