Unable to Create Incident from Interaction in Agent Workspace

Hello all,
Members of our service desk recently reported that the "Create Incident" button on interaction forms in the Agent Workspace is not working. When the user clicks that button, it does bring up a "Create New Incident" tab, but when they click "Save" they get a "Unique Key violation detected by database" "Duplicate entry '<Sys_ID>' for key 'PRIMARY'".
We are using the out of the box UI Actions and configuration for this, and I noticed that when the "Create Incident" button is clicked a blank Incident record is created in the system with that Sys ID that is stated as duplicate in the error, and it does have the appropriate interaction_related_record table record as well.
I'm at a bit of a loss on what could be causing this. Does anyone have any ideas on what could be causing that blank incident record to be created and why that UI Action wouldn't see that record in the interaction_related_record table?

Hello,
Is this issue only on agent workspace and not on native UI?
Can you please go to the Business rule and check if there are any BR, especially before BR which current.update.
If yes then make it false and see if you still get this error.
