Why flow For Each item is not reliable?

Why flow For Each item is not reliable?

I got a simple 'Lookup Records' to get task, then loop it.

The look up records is successfull and has return records, then the 'For each' does not loop it.

i run the same data and its succesful.

Can you share in detail of your issue, what is not working

From the first screenshot I've noticed that the lookup did not return anything.

the 1st and second screenshot is the same run instance, since you cant fit all in one screenshot.

also the issue its the same data was run, first was a failure to loop and the 2nd is successful

Can you Please share what actions are configured in for loop.

regardless of the steps after of Step2: For Each item, it should have loop. you clearly see it but keep asking unrelated steps.
