Why is the menu item I created not showing up in the header menu?

I created a new header menu and added menu items to it.
However, even if you look at the portal screen, the corresponding item is not displayed.
If anyone knows the cause and solution, please let me know.

For this refer OOB "approvals" menu item ( which is a scripted list) . that menu item has this kind of code, you can make changes to code accordingly.
