Workflow activities are getting executed twice

Workflow activities are getting executed twice.
The onboarding workflow for creating new account for user triggering twice due to this generating double request for one user.
1st screenshot(SS1) refers AD creation account fails
1st screenshot(SS2) refers AD creation account success

but both activity are at same time.
can somebody have any idea on it why its triggering twice.

same workflow is getting attached multiple times?

If yes then please check what's the trigger condition for that workflow.

Will check, but this issue is happening for Production Instance only, on Dev & Test it working fine.

unless you replicate you won't know the actual root cause.
