reopen button in employee esc portal

Hi. i want a reopen button in my esc employee portal. i am working on procurement case management. 

Once I received the resolution to the query, I want an option to re-open the case with the option to add the comments and reason for re-open these options should come as a pop up as soon as i click the reopen button. 

i have created the widget, but its not working. please provide me with correct code. i am pasting ss of my code.
