To get this to work, I had to move the variables out of a variable set. From https://docs.servicenow.com/bundle/orlando-it-service-management/page/product/service-catalog-manage..., see this note under container start/split/end:

Variable sets are also considered as containers. So, a container start variable with a two-column layout under a variable set is not supported in Service Portal.
