Ok, I don't think you will find any templates available for onboarding, when your not utilizing the ServiceNow preferred solution. According to your question "I am wondering if there is a document or something that has a list of all the things required from the client to create this onboarding.", I don't believe so. You off course need to have a meeting with the client to understand all facets of their onboarding.
