How to the Resolution Notes and Resolution Code information to appear in to worklogs

Hi,
When we change the state is resloved then the Resolution Notes and Resolution Code should be mandatory then after we will filling the those fields and then after I need to the Resolution Notes and Resolution Code information to appear in to worklogs(Worknotes).
Can anyone please help ont his, It will be helpful.
