Collect Employee Input HR Task type not working / showing blank in the portal

I have created a HR Task Template of type "Collect Employee Input"

And I have selected the employee form

I have created a simple Employee form, with just one question and also no mappings

However when the HR Task is triggered with this template, the user is unable to see the questions in the portal. Simply blank.
Unable to find the Root cause

Hi @Sruthi17,

 

The screenshot you provided is to design the layout of backend forms, not portal (!!!).

 

Review the selected values in the left upper corner to get the idea:

There are table and view selectors and it seems that the field was added to a backend view that has nothing to do with displaying in the Portal/ESC...

This is for the backend and Workspace. But field to be visible on the Portal, you need to adjust it via Record producer or HR Request...

 

Have you done that? Does it make sense?
