Follow these Steps
1. Go to application Navigator and search Flow designer.
2. Now Click on New and then Flow
3.Fill the below fields like flow name, Description and click on Build Flow
4. Now click on Trigger Condition and Fill the details as shown below.
5. Click on Done
6.Now click on Action and fill the details as shown . Also you can put field value as per your requirement.
7. Click On done
8. Save it and Activate it .
9. Now you can test it.
Now you can see it will create incident .
