When an Interaction gets linked to an Incident via interaction_related_record, the Incident's Activity feed it shows: Interaction IMS6338024 'Incident#' field added
The underlying sys_history_line looks like this:
<field>relation</field>
<type>relation</type>
<relation>Interaction IMS6338024 'Incident#' field added</relation>
<update>-1</update>
Not a real Work Notes entry - field/type = relation, update = -1 (not the usual 1/2). None of the interaction_related_record dictionary fields have audit=true, so it's not standard field auditing either.
How do I stop these lines from being added to the Work Notes activity stream?
Appreciate any pointers

ahoy @lonesoac01,
first of all, it might be a good idea to classify the personal details before publicly sharing them.
Also, I don't remember seeing this so it might be related to customisations in your instance. Do you use SN Utils browser extension? If so, you might execute a slash command "/code" with parts of that text to be able locating any script that might be doing that (flow, business rule, script include, client script, ....)
It is difficult to tell you without visibility to the instance

yes @lonesoac01 this is the correct extension, once installed, go to your instance and click "/" (slash) to enable a new popup window and type "code + <message>" and press enter.
In this case that would be "incident#: field added" to try your luck whether some script will be identified of setting this.
So the final slashcommand will be something like:
/code 'incident#': field addedWhat this feature does is searching through all codes whether they contain string " 'incident#': field added ", eventually you can adjust it to something like:
/code incident#or
/code field added
Whatever follows after /code is a string to be searched in the scripts.
The following command will search all the codes having this variable
/code var userGR
This is very powerful feature, if nothing is found then try to get it from different capability, as the text from your comment might be set via message, so you can also check the Messages whether there is something and if you identify a message then you can try it again with something similar to:
/code gs.getMessage('name_of_your_message');
Let me know if there's any progress
EDIT:
you shared the extension exclusively for Chrome but it's available for various browsers, see this for documentation and more info: https://snutils.com/
PS: the whole SN Utils team is full of amazing and sympathetic people :))
