Apply template to description field without wiping out current text

I have a template for my agents to use that goes in the description field to gather user's info

 

"Phone number:

 Office/Remote:"

 

Currently it wipes out any info in the description leaving the technician with no description of the problem. Is there any way around this? I was thinking some code that grabs the current description text and then add the additional text?
