Approval record is not getting created in the approval related list in the RITM

Hi, I'm working on the flow designer and created Ask for Approval of Cost centre manager and the record is not reflected in the RITM table's related list. Any suggestions are appreciated

Is the record not reflected in the related list, or is it not created at all? Knowing that helps with troubleshooting.
